  • Parent Questionaire Spanish

  • Parent/legal guardian photo ID

  • Proof of residence such as a utility, water, or electric bill with your name and address listed. (telephone bills are excluded)

  • Birth Certificate – (Certified Copy) Each child must be five years old on or before August 31 of the current school year

  • Proof of Immunizations - Immunization Record

  • Health Assessment completed and signed by a physician

Immunizations     

Immunizations that are required to enter Kindergarten:

  • Diphtheria, Tetanus, Pertussis (DtaP). Five doses required. 

  • Polio.  Four doses  required. 

  • Measles. Two doses

  • Mumps. Two doses 

  • Rubella. One dose 

  • Haemophilus Influenza Vaccine (Hib). Four doses.

  • Hepatitis B (Hep B). Three doses. 

  • Varicella (chickenpox). One dose.

Rising 6th graders will need a booster dose of the Tdap vaccine if they attend public school and enter the 6th grade after August 1, 2008, if 5 years have passed since the last dose of tetanus/diphtheria
